The revised 3008 form (the Medical Certification for Medicaid Long-Term Care Services and Patient Transfer Form) is being incorporated into a new Medicaid rule, chapter 59G-1.045. A proposed rule hearing was held July 16, 2015 and it is expected the new rule will be final by September or early October 2015.
